I remember reading about that specific incident. If I am not mistaken it happened at night and as far as I recall the guy in the back seat was not a pilot, but a doc or mechanic or something.
Don't recall the exact cause, but I think you are on the right track. An american F-16 pilot who ha access to the information would be helpfull. We see the american reports, but I am not sure if we store them after we are done with them.
